Changes in version 0.12                         

  - add new functions calculate_CI_I_pp and CI_np_bs to calculate
    confidence intervals for the gren estimate of incidence per patient
  - add new function create_patient_history_add to simulate a timeline
    of a bed
  - add "naive" estimator based on empirical probability of A=1 to the
    set of estimators for calculate_I and generate_I_fast
